In order to play in the Wentworth Community Ball Program, a completed registration form AND Parent's Code of Conduct form must be MAILED into the Wentworth Ball Program or hand delivered to the school along with your payment. The address is listed on the registration form. Both forms MUST be filled in completely and MUST be signed by the player's parent or legal guardian. ALL volunteers MUST fill out a volunteer form before working

Incomplete forms, registraion forms mailed without a Parent's Code of Conduct form or registraion forms mailed without payment WILL NOT BE ACCEPTED.
